Guide For Families

We provide a true Patient Centered Medical Home for our patients–infants, children, teens and young adults from birth to twenty-one years of age. We work as a team with the patient, parents and other providers to provide excellent pediatric care using evidence-based guidelines. We care for the whole child in the context of his/her family, school, and community. We provide excellent pediatric care–well child care, acute illness assessment and treatment, chronic illness care and follow-up, and specialist referrals as appropriate.

What is the Patient-Centered Medical Home (PCMH)? 

As defined by the American Academy of Pediatrics the  PCMH is a pediatric practice where “… the medical care of infants, children, and adolescents ideally should be accessible, continuous, comprehensive, family centered, coordinated,1 compassionate, and culturally effective.2 It should be delivered or directed by well-trained physicians who provide primary care3 and help to manage and facilitate essentially all aspects of pediatric care. The physician should be known to the child and family and should be able to develop a partnership of mutual responsibility and trust with them.” (AAP policy statement 2002, reaffirmed 2008)

As a PCMH, New Britain Pediatric Group is the primary resource for all of your child’s health care needs. In a PCMH the parents and child as well as your child’s doctor, nurse, patient care coordinator, medical assistant and medical receptionist all work together as a healthcare team.  Parents and families are a respected, valued and are an integral part of the healthcare team.

We will provide needed information and help patients and families develop care/treatment plans, and help co-ordinate care across all medical settings including:

New Britain Pediatric Group is accessible 24 hours a day 365 days a year through our office, website, patient portal and on-call physician coverage.  We are here to offer expert pediatric advice and care whenever it is needed.  Just call our office any time of day, if it is after hours, you will be transferred to our answering service who will contact the on-call providers.

Well care appointments, follow-up appointments and sick visits can all be scheduled during office hours 9 AM to 4:30 PM Monday-Friday and 9 AM to noon on Saturday mornings by calling the office or accessing the electronic Patient Portal.  Same day appointments are available.  Extended hours are also available Sunday morning for urgent problems and newborn follow-ups as needed.

You can expect the following from your PCMH:

  • We are also available by telephone and through our electronic Patient Portal to answer medical questions, discuss parental or patient concerns, refill prescriptions, answer billing questions and appointment requests.
  • We will notify parents/patients of laboratory and test results on a timely basis.
  • We will refer your child to a specialist (and expedite the process) if needed after appropriate discussion with discussion with you and with your permission. We share information with specialist and with you in a timely manner.
  • Requests for special accommodations may be made through any physician or Manager.

As Your Patient Centered Medical Home We Will:

Provide ongoing well child care to help you keep your child healthy, monitor growth and development, support your role as a parent and help you set goals for your child’s care.

  • Take care of your child when he or she is sick and help you to manage acute illness as well as chronic conditions.
  • Discuss with you any testing or treatment that your child needs.
  • Work with you and other care providers, including specialists, to coordinate care.

You Can Make the Most of Every Visit By:

  • Sharing how your child is doing–at home, play and school.
  • Writing down and sharing your questions and concerns.
  • Providing a contact list of all other care providers that your child sees.
  • Informing us of any Emergency Room or Urgent Care visits.
  • Sharing a list of medications ordered by other providers.
  • Asking for help in creating and sharing a complete record of your child’s care.

Communication is the Key to Providing Excellent Care

Every patient and family in our practice is part of their own care team and is encouraged to have open communication with their healthcare provider and all staff.

All medical providers and staff can be reached by telephone or through the electronic Patient Portal, which may be accessed by registering through our website, www.newbritainpedi.com and clicking on the Patient Portal tab.
